New State Regulations -Who Has Access & How to Obtain a Vital Certificate (effective September 28, 2011)

Checks are payable and mailed to the Town of Bar Harbor, 93 Cottage St., Suite I, Bar Harbor, ME 04609.
Complete applicable application and attach a second page with a copy of required identification(s). Identification will be shredded upon completion of
request.
Certificates may be obtained from:
1. The city or town in which the event occurred,
2. The city or town in which the individual resided at the time of the event,
3. The Office of Data, Research and Vital Statistics, 244 Water Street, #11 SHS, Augusta, ME 04333-0011. Phone 1 888 664 9491 or 207 287 3181.
Request maybe made by application or in-person, payment must be received at the
time of request along with the name at the time of the event, date of
event and relationship to the person named. If other than oneself, proof of direct or legitimate interest must be provided. Proof of identification is now required whether record is requested in-person or by mail.
The fee for a certificate obtained from a municipality is $15.00 for the first copy and $6.00 for any additional copies.
The fee from the Office of Vital Statistics is $15.00 for the first copy and $6.00 for any additional copies. The State does accept credit cards.
